Calaveras Commision on Aging seeking 'Senior of the Year' nominations

Senior volunteers are a vital lifeline for many of the churches and organizations in this county.  Help us honor their efforts by nominating them for the Senior of the Year.  All nominees are awarded certificates of appreciation and acknowledged by the supervisor of their district.
 
To nominate a senior, please complete the attached form, or feel free to use your organization’s stationery, to explain the reasons for nominating each person. We encourage you to work with your nominee to be sure you include their service to not only your organization, but any others.   Please include how long the volunteer has been associated with your organization, the name of your organization, and details of the contributions made.  Please type or print. Do not use abbreviations.  You may use additional sheets if necessary. 
Only information submitted may be used to select the winners; therefore, please document all their volunteer activities in Calaveras County.
Work with your nominee - the more you can document the better.
 Eligibility Requirements:
  1. A resident of Calaveras County.
  2. 60 years of age or older.
  3. An active volunteer in the County. 
    (They do not need to be members of your organization.)
  4. Not a previous winner.
