The VA Rural Health Mobile Medical Outreach Team will be at:
San Andreas Senior Center
956 Mountain Ranch Road
San Andreas, CA 95249
Thursday, November 10th
10:00am – 2:00pm
No appointment necessary!
A medical and mental health team will be on site.
- A Rural Health Team will provide examinations and consultations, referrals, and prescription renewals.
· If you are uncertain about your eligibility for medical or psychological services, members of our team can provide you with information about and assistance with eligibility and enrollment for VA care. VA healthcare may complement your current insurance coverage. Eligibility requirements have changed, therefore, if you have been denied in the past, please come and meet with us. Benefits are available for Vietnam and Iraq/Afghanistan Veterans.
· If you are NOT currently in the VA system, bring a copy of your DD214 to attach to the enrollment form. If you do not have a copy of your DD214, we will obtain one for you.
· In addition, volunteer Veteran Service Officers will be available to review your benefits and to provide assistance with filing claims.
